This form must be completed before scheduling your equine’s first session. This helps customize the bodywork to your horse’s individual needs and maximize the benefits of their session.

I, Erica Jones, Footprint to Hoofprint Equine Bodywork & Services am not a veterinarian. I do not diagnose or treat medical issues, prescribe drugs nor perform surgery. My role is to support your animal(s) (along with veterinarian, dentist, etc.) in order to improve or maintain their wellbeing. The specific sessions will be different for each animal.

I will NOT perform bodywork on animals with the following conditions:

· Pregnancy (unless directed by a veterinarian)

· Recent operations or acute injury (unless directed by a veterinarian)

· Fever- usually indicative of general or local infection

· Skin Disease – fungal or bacterial, including rain root

· Cancer, cardio-vascular problems, hemorrhaging following accident

· Neurological disorders (unless directed by a veterinarian)

· Colic rehab (unless directed by a veterinarian)

· Horses that have been administered joint injections, vaccinations or de-wormer within the last 5 days

By checking this box I agree that I have requested equine bodywork from Footprint to Hoofprint Equine Bodywork & Services and give permission to Erica Jones to touch and perform bodywork on my animal(s).

By checking this box I agree to accept financial responsibility for any bodywork performed by Footprint to Hoofprint Equine Bodywork & Services ( cash or check). I understand that there is a $50 fee if my animal(s) must be caught/groomed/dried or if I cancel within 24 hours. If the horse is frustrated or agitated during the massage and isn’t relaxing, the bodywork will be terminated. It is requested that the owner work with desensitizing the horse to being touched all over before another session is scheduled. A trip/gas charge will be applied. A reiki session can be substituted.

By checking this box I agree that Under Florida Law (Ch. 773 Equine Activites), an equine activity sponsor or equine professional in not liable for an injury to or the deith of a participant in equine activities resulting from the the inherent risk of equine activites.Owner agrees to hold harmless and indemnify Erica Jones, Footprint to Hoofprint Equine Bodywork and Services, LLC., from and against any and all claims, demands or lawsuits arising out of or related to bodywork of animal.

By checking this box I certify that I have read and agreed to the following: Bodywork Expectations & Service Area: In order to keep both myself and your horse safe, I will not preform bodywork sessions in a lightning storm, at feeding times, stalls or hard tied/cross tied. The horse must be in a comfortable, clean, dry quiet shelter or pasture/paddock. Please walk or turnout your horse at least 10 minutes before their session. Footprint to Hoofprint Equine Bodywork & Services is based in Spring Hill FL and services the Tampa Bay Area North. If you are located more than 20 miles from my home base there will be an additional travel fee. Please contact for specific pricing. I also require equine’s owner to be present for the entire first session which may take up to 2 hours ( due to paperwork and assessements).
